Most of the review time goes on working out what a compiled class is doing. When the source is published and matches the artifact, that work largely disappears, so there is nothing to bill for. If the published source turns out not to match the build, the submission moves to the paid tier and we tell you why.
The cost is review time, and review time is spent per build. A subscription would overcharge anyone shipping twice a year and would put pressure on us to hurry when someone ships weekly.
The badge changes to expired and the verification page says the same. Re-certifying an artifact we have already reviewed is free, since the review is on file. A rebuilt artifact has a different hash, so it is a new review.
There is nothing to refund, because you are charged on issue. A rejected build costs you the upload, and you get the reviewer's notes on what would need to change.
Not as one unit. A pack is a list of other people's artifacts, and certifying it would mean attesting to work we did not review. The individual mods inside a pack can each hold their own certificate.
